Consumer health data we collect
- information about disability and the accommodation requirements you disclose to enable your participation in an event, program, or activity including, but not limited to, step-free access, sign language interpretation, captioning, sensory requirements, or dietary requirements as related to a health condition; and
- information that pertains to your health that you include in a communication sent to us.
Why we collect and use it
We will use your consumer health data solely for arranging the accommodation you requested, making the event or program that you have requested accessible to you, and responding to your request. Your consumer health data will not be used for any marketing, fundraising, or advertising purposes, and we will not sell it.
Where it comes from
- from you, via email, phone call, an event/program registration form, or face-to-face contact; and
- from the person that you ask to make a request on your behalf, such as your caregiver, support person, or chapter representative.
Consumer health data we share
We share only the information about your accommodation that is needed to arrange your accommodation needs, such as “need access to the facility without steps,” or “request American Sign Language interpreter.” We don't share your medical information unless you ask us to.
Who we share it with
- Provided with your consent: Event location sites, chapters or other organizations participating in organizing the event, as well as accessibility service providers (interpreters, captioners, transportation providers). We will not provide this information to them without your consent.
- Information processors that work for us: our e-mail and web-based form services, that store and deliver the information on our behalf.
- Affiliates: Passport to Change does not have any affiliates receiving consumer health data.
Your rights and how to use them
You have the right to:
- ascertain if we collect or share your consumer health data and request its copy, along with a list of all third parties and affiliates we shared it with and their contact information;
- revoke your consent; and
- have your consumer health data deleted.
To make a request, send an email to info@passporttochange.org under the subject line “Health data request.” We may request information from you to verify your identity. Within 45 days, we will get back to you. In case we need additional days, we will inform you about that, and why, within 45 days and could take up to 45 additional days. Requests are free up to two times per year.
Along with erasing your consumer health data, we also notify all processors and third parties that received the information to do the same.
In case of denial of your request, an appeal is possible by responding to our decision using “Appeal.” We will respond to your appeal in writing within 45 days. In case your appeal is denied, we will explain how to file your complaint with the Washington State Attorney General.
